📝 Undertaking & Declaration | Sports Circle

This Undertaking is made by the undersigned (“I”, “me”, or “the participant”) in favor of Sports Circle, its management, employees, coaches, representatives, and partners (“the Club”).

By signing this document or submitting my registration digitally, I hereby declare that I have read, understood, and voluntarily agreed to abide by all the rules, regulations, and policies of Sports Circle, including but not limited to its Terms & Conditions, Privacy Policy, and Indemnity Policy.

1️⃣ Acknowledgement of Risk

I understand that participation in sports, training sessions, and physical activities at Sports Circle involves inherent risks, including but not limited to:

Physical injury, muscle strain, or accidents
Health complications or medical emergencies
Damage or loss of personal property
Risks associated with equipment or playing conditions

I voluntarily choose to participate in such activities entirely at my own risk and confirm that I am physically fit and medically capable of doing so.

2️⃣ Health & Medical Declaration

I confirm that I am in good health and free from any condition that may prevent safe participation in sports or physical activity.
I will immediately inform the Club staff of any injury, medical issue, or discomfort experienced during play.
I understand that Sports Circle does not provide medical supervision and any first aid or emergency response provided is on a good faith basis only.
I agree to bear full responsibility for any medical treatment, expenses, or outcomes arising from such participation.

3️⃣ Compliance With Club Rules

I agree to follow all rules, safety guidelines, and instructions provided by Sports Circle staff and coaches.
I will wear proper sports attire and use equipment responsibly.
I will respect the facility, staff, and fellow members at all times.
I will not engage in any activity that causes damage to property or endangers others.

Sports Circle reserves the right to deny or terminate my access in case of misconduct or rule violation without any refund.

4️⃣ Waiver of Liability

I understand and agree that Sports Circle, its management, and staff shall not be held responsible for:

Any personal injury, illness, or accident arising from participation
Loss, theft, or damage of personal belongings
Any indirect or consequential losses of any kind

I hereby waive all rights to claim or demand compensation from Sports Circle or its representatives for any injury, loss, or damage incurred during or after participation in any activity.

5️⃣ Indemnity Clause

I agree to indemnify, defend, and hold harmless Sports Circle, its owners, employees, affiliates, and representatives from any and all:

Claims, actions, damages, costs, expenses, or legal proceedings
That may arise directly or indirectly from my actions, participation, or negligence

This includes — but is not limited to — damages caused to equipment, property, other participants, or third parties during my use of the Club facilities.

6️⃣ Photography & Media Consent

I grant Sports Circle full permission to use any photographs, videos, or recordings taken during my participation for marketing, promotional, or social media purposes, without any financial or personal claim.

I understand that such content remains the intellectual property of Sports Circle.

7️⃣ Property Damage Responsibility

In case I accidentally or intentionally damage any facility, equipment, or property belonging to Sports Circle, I agree to bear the cost of repair or replacement as determined by the Club management.

8️⃣ Refund & Booking Acknowledgment

I understand that:

All bookings at Sports Circle follow a Pay & Play policy.
Cancellations or no-shows are non-refundable, except for operational closures or rescheduling by the Club.
Management decisions regarding refunds, re-bookings, or compensations are final and binding.

9️⃣ Governing Law

This Undertaking shall be governed by and interpreted in accordance with the laws of India.
Any disputes shall be subject to the exclusive jurisdiction of the courts located in [Insert City/State of Registration].

10️⃣ Declaration

By signing this Undertaking, I confirm that:

I have read and understood all the clauses stated above.
I am entering this agreement voluntarily and without any coercion.
I assume full responsibility for my safety and actions while using Sports Circle’s facilities.
I have no objection to the Club enforcing its rules and safety guidelines at its discretion.

I, the undersigned parent/guardian, confirm that I have read and understood this Undertaking on behalf of the minor named below.
I accept full responsibility for the minor’s participation, conduct, and safety while using the Club facilities.
